SMS Terms and Conditions for AthleticsMD®

1. SMS Consent Communication
Phone numbers collected for SMS consent will never be sold or shared with third parties for marketing.


2. Types of SMS Communications
If you opt in, you may receive:

  • Appointment reminders and confirmations
  • Lab result notifications (no PHI sent in SMS; portal links only)
  • Prescription refill and follow-up reminders
  • Billing and payment notices
  • Limited promotions or offers related to AthleticsMD® services

Example:
“Hello, this is a reminder of your upcoming appointment with AthleticsMD® on [Date] at [Time]. Reply STOP to opt out.”


3. Message Frequency
Frequency may vary by service. Patients may receive up to 5 messages per week related to appointments, billing, and follow-ups.


4. Fees
Standard message and data rates may apply depending on your mobile carrier.


5. Opt-In Method
You may opt in by:

  • Checking the SMS consent box on online or paper intake forms
  • Verbally agreeing during intake
  • Texting AthleticsMD® first through our enrollment number


6. Opt-Out Method
You may opt out at any time by replying STOP to any AthleticsMD® message. You may also contact us at 844-FIT-MD4U to be removed.

  • AthleticsMD® is not responsible for adverse effects resulting from undisclosed medical history, allergies, or non-compliance with care plans.
  • Patients agree to indemnify and hold harmless AthleticsMD® and its staff against claims arising from misrepresentation of medical history or unauthorized use of educational materials.
